The Role of PRP and Mesotherapy Treatments in Hair Loss Treatment

Hair loss is a condition that significantly affects people's self-confidence and quality of life. Especially alopecia areata and androgenetic alopecia are the most common types of hair loss. There are different treatment methods in the treatment of these diseases.

The most commonly used topical minoxidil in the treatment of androgenetic alopecia, twice a day is a laborious application and there is a risk of irritation. Sudden discontinuation of minoxidil causes hair loss again. Systemic antiandrogen drugs are not recommended because of their high side effects. Oral finasteride has been proven to be effective in male patients, but its use in female patients is controversial. Medical treatments may not always be effective in these diseases and patients are constantly in search of new treatments. Mesotherapy and Platelet Rich Plasma (Plateletrich plasma, PRP) methods have been used in cosmetic dermatology with increasing frequency in recent years.

Mesotherapy; It is an injection process applied to problem areas with very small drug doses. The drugs used are often natural plant extracts, homeopathic agents, vitamins, minerals and pharmaceuticals. Mesotherapy method is used in cosmetic dermatology, rheumatology, sports medicine, neurology. The most important advantages of this minimally invasive procedure are treatment with small drug doses and protection from systemic side effects of drugs. Regardless of the type of shedding in hair loss treatments, the treatment protocol is usually the same. It is applied in the form of multiple intradermal injections made on the scalp. It starts with 1 session per week for the first 4 weeks and then monthly maintenance treatments are recommended. The number of maintenance treatments may vary from case to case. Side effects of mesotherapy applied to the scalp are generally tolerable. Headache, itching, and erythema are common side effects and go away within a few days. Infections, foreign body granuloma, ulcers, scar formation can be seen due to incorrect injection technique or mesotherapy applications performed under inappropriate conditions. Hypersensitivity reaction due to the drugs used is also among the rare complications.

Platelet-rich plasma (PRP, platelet-rich plasma) means increased platelet concentration in the small volume of the plasma. PRP contains many growth factors in protein structure that are released from platelets and are involved in wound healing. This treatment model has gained importance as a result of the discovery of the effects of platelet-derived growth factor (PDGF) on wound healing, vessel formation and tissue regeneration. Platelet-rich plasma contains highly concentrated platelets. The cocktail of these growth factors causes tissue repair and regeneration. Activated PRP provides proliferation and differentiation of stem cells in the hair follicle through many molecular mechanisms. PRP can also be used in combination with the mesotherapy technique in the treatment of androgenetic alopecia. At the end of the treatment, it was determined that there was a noticeable increase in hair diameter and hair density.

In general, it can be said that mesotherapy and PRP application increase hair density and quality in androgenetic alopecia. Since it is a minimally invasive and safe procedure, mesotherapy and PRP applications are increasingly preferred in androgenetic alopecia. However, it cannot be considered as a superior treatment to hair transplantation due to the lack of standard protocols regarding the number and frequency of applications, centrifuge methods, injection sites, injection depth and the amount of treatment that should be given in each session.
